We have recently been engaged by Fareham and Gosport CCG to help with establishing the Hampshire and Isle of Wight ICS. We are leading on stakeholder engagement, building the vision, and will use NELIE for analysis, benchmarking of current performance and modelling of new models of care.
Areas for improvement have been identified, and we are conducting deep dives using our population health and patient segmentation systems and proprietary academic models from King’s College London and the US.
Our partners, OptiMedis Cobic, will work with us on the development of an outcomes framework and the business case for realising the vision of an ICS. University College Health Partnership will bring expertise in data intelligence and evaluation of interventions.
This system-integrating pilot covers a population of approximately 200,000 people, with the evaluation and close expected in July 2018. It is then planned to scale the work across the whole ICS population.
