Continuing Healthcare

We provide a comprehensive service, enabling commissioners to make decisions regarding Continuing Healthcare and Funded Nursing Care for Adult and Continuing Care for Children. The Continuing Healthcare Service provides an end-to-end service for all phases of Continuing Healthcare and Continuing Care management; inclusive of:  clinical assessment to support eligibility; decision making and ratification processes; brokerage of packages of care; contracting; quality; invoicing; and business intelligence. 

As part of the wider CHC Service, we also offer:  Personal Health Budgets (PHBs); Appeals and Retrospective Claims and a Historic Claims service, for previously unassessed periods of care, in-line with the Department of Health guidance.

Different service models are provided to individual CCGs, dependant on the local service model required.

The CHC Service work to Standard Operating Procedures, with underpinning strong leadership, governance, quality assurance, safeguarding and professional practices.  The Continuing Healthcare Nurses and Practitioners employed by the service are highly skilled professionals with extensive knowledge and expertise across the assessment and commissioning functions provided. Using a single database we ensure that the National Framework is met; such as the 28-day assessment process timescale, and three and twelve month review timescales. Our teams have facilitated excellent working relationships with stakeholders, allowing us to maximise our efficiency and effectiveness.

The CHC Service has a strong ratification process for decision making, which supports the CCGs to assess funding and placement options; alongside supporting customers to effectively manage provider performance and quality at a strategic level.
