Medicines Support Service

The Medicines Support Service is for adults in Norfolk, who have problems managing their medication. Its aim is to facilitate care of people in their own home by providing a professional medicines management assessment and support service. 

Referral into the Medicines Support Service

The person needs to be over 18, registered with a Norfolk GP, and there is a dosage regime where compliance is paramount and in addition, at least one of the following criteria is met:

  • Confusion with medication
  • Difficulty managing medication
  • Due or recent discharge from hospital
  • Other exceptional circumstances agreed with Service Co-ordinator

Referral into the Service comes through the patient’s pharmacy or dispensing surgery.

Any health professional, social worker, carer, relative or patient can approach the pharmacy or dispensing surgery for an initial assessment under the Equality Act and onward referral to the CSU service by the pharmacy or dispensing surgery if they are unable to help.
