Supporting change to implement innovative new models across an STP

We were asked to support the delivery of priority workstreams within North London Partners in Health and Care (the STP for north and central London)

Our team is providing programme support to the Urgent Emergency Care and End of Life Care workstreams and is project managing the implementation of supported discharge models in stroke care.

In addition, we are working with clinicians from across the system to agree new models for planned care, with a specific focus on the urology pathway and the development of a clinical advice navigation service.

Despite support only just commencing, we are already:

  • Making progress on agreeing new models of care for end of life care and supported discharge within stroke and we are drafting service specifications for new models of care
  • Helping to increasing the levels of engagement with priority workstreams among clinicians and partner organisations within the STP
  • Making a compelling case for investment in end of life care nursing to improve quality and sustainability
  • Increasing the pace of delivery on agreed system-wide challenges
